It is our goal to guide you through today's complex world of medical billing. Our office will prepare and file your insurance claim forms for you based on the information you provide to us. Please bring your insurance card to your visits so we may process your claims accurately.​

​We are in network with the following insurances:

  • Aetna - HMO/PPO We are not in network with EPO or Baylor Scott & White Aetna Plans (yellow cards)
  • Blue Cross Blue Shield - PPO 
  • Cigna - HMO/PPO
  • United Healthcare - HMO/PPO
  • ​Molina Marketplace 

There are a few other small insurance companies we are contracted with if you think your insurance may be one of them, please email a copy of the front and back of your card to and we would be happy to verify if we are in network.

  • You will receive a text message containing a link. Click on link to visit the Zoom web page
  • Download the Zoom client for meetings
  • Right before your scheduled appointment time, click on the link again from the text message.  
  • That will take you into the ZOOM meeting (waiting room) where your provider will be.

We utilize ElationPassport to securely manage your health and communicate with our office. When you get established as our patient, we will send you an invite to create your Passport Portal account. Benefits of using this tool include:

  • ​View medical provider notes from your visits
  • Securely message our office
  • ​Access your patient information
  • ​Available on Mac, PC and mobile

New Patient Paperwork will be emailed to you once you call and schedule your new patient appointment.  We must have your paperwork completed and emailed back to us at least 48 hours before your appointment. This will assist us in serving you as efficiently as possible.

Failure to complete prior to appointment will result in rescheduling your appointment.

**If you need to reschedule your appointment be sure to contact our office 24 hours before your scheduled appointment to avoid a $25 Same Day Cancellation Fee or a $50 No Show Fee. This will be in effect starting January 1, 2018.**
